How much does treatment cost?
Prolotherapy, PRP, and Stem Cell treatments are not covered by any insurance provider. Woz Wellness is an an out-of-network provider.
The cost of Prolotherapy treatment varies primarily based upon the area(s) that are being treated, the complexity of treatment, the substance(s) that are used in the treatment, and how many treatments are needed.
For example, a single knee treatment using dextrose would cost less than a bilateral knee treatment using Platelet Rich Plasma (PRP) or stem cells taken from your bone marrow.

Ultrasound guidance is always included in injection pricing.
The best way to determine how much a treatment will cost is by booking a New Patient Consultation with Dr. David Woznica.
In this appointment Dr. Woznica will review your specific musculoskeletal pain disorder(s), determine if you are a good candidate for Prolotherapy, create a custom Treatment Plan, and fully explain all costs.
While many patients feel pain relief after their first Prolotherapy treatment, in order to fully heal an injury it typically takes three to six treatments on average.
There will never be any surprises or hidden costs when receiving a treatment at Woz Wellness.
Standard Payment Policy
Payment is expected at the time of treatment.
New Patient Consultations are always required for new patients prior to any Prolotherapy, PRP, or stem cell treatment.
New Patient Consultations may be done in person or virtually.
New Patient Consultation cost is $350 and is non-refundable. However, this fee is applied to the cost of your first treatment if started within 2 weeks of your New Patient Evaluation.
no show / cancellation fee
No-shows and cancellations made within 24 hours of your scheduled appointment will be charged a $50 fee to the credit card given on file at the time of booking.
insurance Claims
Prolotherapy, PRP, and Stem cell treatments are not covered by any insurance provider. Prolotherapy and similar types of regenerative medicine injection therapies are still viewed as experimental treatments. This is not due to a lack of evidence that Prolotherapy works, rather simply due to the lack of research and clinical trial studies that are accepted or large enough to compel insurance companies to cover the procedure.
Upon request, at no additional cost, Woz Wellness can provide you with an itemized statement that you may submit to your insurance company for possible reimbursement.
The itemized statement will be marked as paid, allowing the insurance company to reimburse you directly.
While we can provide itemized statements to be submitted, Woz Wellness is not within any network and in no way can guarantee any level of insurance reimbursement.
Medicare/ Medicaid
Woz Wellness is not a Medicare or Medicaid provider and claims for services received by Dr. David Woznica at Woz Wellness cannot be sent to Medicare or Medicaid.
Legal Cases
Woz Wellness does not take workman’s compensation, auto accidents, and legal cases.
If you choose to receive treatment at Woz Wellness you will be required to pay 100% at the time of service and will receive receipts for all services and payments.
You as the patient are responsible for filing your own claims.
Special Documents & consult Requests
Woz Wellness will always supply itemized statements and simple letters for post procedure return to work or school at no additional cost.
Additional fees may apply for requests such as:
• Phone consultations that do not involve questions about immediate post-treatment care require pre-scheduling of a 30 minute period. Cost is $75 and is paid at time of scheduling. Click here to email us and schedule a phone consultation.
• Advanced letters or extensive documentation costs will be quoted at the time of request. Click here to email us and request a quote.
• Imaging review done outside of a scheduled visit (via phone call, text, or email) of medical imaging done by third parties such as X-Rays, MRI’s, Ultrasounds, etc. that were not ordered by Dr. Dave cost between $175-$300. Cost will be given at time of request and is based on complexity of case. Imaging review done during scheduled visits are at no additional cost. • Any “Emergency” / “Response Needed ASAP” Requests that require special immediate attention, documentation, opinion, or consult with Dr. Dave within 24 hours incur an additional charge of $25. This is in addition to fees outlined above. (This does not include immediate post-treatment care requests, support, or questions that are always provided as soon as the request is received.)

Payments Accepted
Woz Wellness accepts all major credit cards, HSA/FSA funds, and/or cash for payment.
You may decide to use Flexible Spending Accounts (FSA) Funds or Health Savings Accounts (HSA) funds for Prolotherapy, PRP, stem cell injection treatments, or other services performed.
Some patients find this is an excellent way to use their non-taxable health care funds for medical care outside of their network plans.
